The Social Worker will assess patients and their families to identify social, emotional, and cognitive needs while coordinating necessary community resources. They are responsible for maintaining accurate documentation of assessments, treatment plans, and evaluations to support patient care.
Candidates must hold a Master's Degree in Social Work and possess the appropriate state licensure or be eligible for supervised practice. Previous experience in a hospital or acute care setting is required for this role.
