Key Responsibilities

The Ambulatory Care Clinical Pharmacist provides specialized drug therapy management, develops individualized monitoring plans, and collaborates with healthcare providers to optimize patient outcomes. They are responsible for medication error prevention, patient education, and coordinating complex medication therapy including prior authorizations and financial assistance.

Requirements

Candidates must be graduates of an ACPE accredited College of Pharmacy with a current Oregon pharmacist license or eligibility. The role requires completion of an ASHP accredited PGY-1 residency or 5 years of relevant clinical pharmacy experience, with rheumatology experience preferred.
