Key Responsibilities
The Nurse Practitioner will provide professional and comprehensive care to critically, chronically, or terminally ill patients in an interdisciplinary setting. Responsibilities include conducting home visits for patient recertification and maintaining accurate, timely clinical documentation.
Requirements
Candidates must hold a current Registered Nursing license and a Nurse Practitioner license in the State of California. A minimum of one year of professional nursing experience is required, with hospice or palliative care experience preferred.
