Job description
Join Mission Hospice and Home Care as a Per diem Hospice Triage Nurse - Remote - MUST HAVE CA RN LICENSE in San Mateo, California.
Key Responsibilities
- They will assess the needs of the patient and family, and coordinate the needed care and/or scheduling of home visits as needed.
- The triage nurse will collaborate with the transdisciplinary team members and facilitate communication among the transdisciplinary team, Hospice, and the patient’s Attending Physician.
