The case manager will provide therapeutic behavioral services and develop individualized treatment plans for clients. They are also responsible for coordinating care, facilitating linkages to services, and performing crisis prevention interventions.
Candidates must have a high school diploma with three years of relevant experience or a bachelor's/master's degree in a related field. A valid driver's license and reliable transportation are required for travel within the service area.
