Performs all other duties as assigned.

Key Responsibilities

• Gathers appropriate data and formulates relevant assessment of patient and family’s medical, social, and emotional situations.

• Gathers data reflective of the patient and family’s developmental stages.

Requirements

• Escalation required within 90 days of hire and annually thereafter.

Benefits & Compensation

• Salary: $88,629 - $103,834
