Provide specialized Physical Medicine and Rehabilitation services to patients within a skilled nursing facility. Collaborate with interdisciplinary teams, including physical and occupational therapists, to optimize patient functional outcomes and independence.
Must hold a valid Nurse Practitioner, APRN, or Physician Assistant license in Texas. Experience in the post-acute or skilled nursing facility space is preferred.
