Nurse Practitioner

Key Responsibilities

The Nurse Practitioner will travel to nursing homes and assisted living facilities to provide initial evaluations, therapy services, and cognitive testing for patients with neurocognitive disorders. They will also document care, maintain professional relationships with facilities and families, and work within a multidisciplinary team to promote patient safety and functionality.

Requirements

Candidates must hold a Master’s degree in a Nurse Practitioner specialty and possess current state licensure and ANCC or AANP certification. At least one year of clinical APRN experience is preferred, along with proficiency in EMR systems and strong interpersonal skills.
