Key Responsibilities
Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, to bridge gaps in care. Build trusted relationships with patients and provide connections to appropriate healthcare services based on unique needs.
Requirements
Requires an active, unrestricted medical license (MD or DO) in the coverage area. Multi-state licenses are preferred for this full-time clinical role.
