Job Description
Empower Recovery. Lead with Care. Redefine Post-Acute Medicine.
Are you a clinician who believes that the transition from hospital to home is the most critical window in a patient’s journey? Do you thrive in environments where you can build deep relationships, lead interdisciplinary teams, and practice medicine at the highest level of your license?
Eterna Primary Care is looking for a Medical Practitioner (NP/PA) who is passionate about geriatric and post-acute care to join our mission-driven team.
The Perks: Why You’ll Love Eterna Primary Care
Work in a highly collaborative, team-based environment with nursing staff, therapists, and facility leadership focused on patient-centered care.
Partner closely with physicians and consultant pharmacists, gaining exposure to interdisciplinary clinical decision-making.
Receive ongoing mentorship from experienced physician leaders to support your clinical and professional growth.
- Be part of a culture that values collaboration, learning, and continuous improvement in patient outcomes.
- We value our practitioners not just as employees, but as clinical leaders. To support your lifestyle and professional growth, we offer:
- Competitive Pay: Earn a high-impact salary with compensation up to $170K.
- Comprehensive Benefits: An excellent package, including CME reimbursement to keep your skills sharp.
- True Autonomy: Enjoy the independence to make clinical decisions and the flexibility to manage your schedule.
Efficiency by Design: Experience seamless EMR documentation and the advantage of dedicated support staff who drive efficiency, allowing you to focus on the patient, not the paperwork.
Rewarding Outcomes: See the direct impact of your work with performance gains realized under our innovative value-based care model.
The Role: Why This Matters
At Eterna Primary Care, you aren’t just "rounding", you are the primary medical architect for patients in skilled nursing and assisted living facilities. You will bridge the gap between acute crisis and long-term wellness, ensuring that every patient receives high-quality, patient-centered care designed to reduce hospital readmissions and improve quality of life.
Ready to Make a Difference?
If you are ready to move away from the "revolving door" of acute care and into a role where you can truly manage a patient’s path to recovery, apply today!
Eterna Primary Care is an equal opportunity employer committed to a diverse and inclusive healthcare environment.

What You’ll Do

Focus on Value: Champion quality metrics and cost-effective medicine that prioritizes the patient’s goals over everything else.
