Conduct comprehensive in-home health assessments for Medicare Advantage members, including physical exams and medication reconciliation. Collaborate with primary care providers to address gaps in care and educate members on disease processes and social determinants of health.
Requires active ANCC/AANP certification for NPs or NCCPA certification for PAs, along with unrestricted state licensure and a valid driver's license. Candidates must have access to reliable transportation and the ability to navigate varied home environments to perform assessments.
