Conduct comprehensive in-home health assessments for Medicare Advantage members, including physical exams and medication reconciliation. Collaborate with primary care providers to address care gaps and educate patients on disease processes and social determinants of health.
Requires an active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care. Must possess an unrestricted driver's license and the ability to travel to various home environments.
