Job description

Join Maryland Care Management as a Social Worker (SDoH) in Howard/Montgomery/Prince George's County, Maryland. *Candidate must live in/be familiar with one of the following counties in Maryland: Howard, Montgomery, Prince George's* Summary: Maryland Care Management, Inc. is searching for an innovative Social Worker located in Maryland who is a self-starter with a problem-solving mindset.

Key Responsibilities

  • Utilizes clinical judgment to assess members, prioritizing emerging issues to maintain a member-centric approach.
  • Collaborates with interdisciplinary care team to aid member health goals via case rounds and consultation.
  • Utilizes problem-solving skills to research and identify community resources and coordinate referral mechanisms.
  • Plans specific goals and actions designed to meet the member’s needs as identified in the assessment process that are action-oriented, time-specific, and cost-effective.

Requirements

  • Knowledge of community agencies and resources. (PG/MOCO).
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.

  • Full-Time position.
  • Hybrid position - combination of remote and on-site.

This position offers hybrid flexibility. Work with Maryland Care Management in Howard/Montgomery/Prince George's County, Maryland.

Licensure & Certifications

Requires a Bachelor's degree in social work, a valid Maryland LMSW or LCSW license, and 3 years of community setting experience. A valid Maryland State Driver's License is mandatory, and a Master's degree is preferred.
