Key Responsibilities
Provide primary and acute care via telephone and video to patients in home and community-based settings. Develop and implement complex clinical plans of care while collaborating with physicians and interdisciplinary teams.
Requirements
Requires a Master of Science in Nursing, national NP certification, and an active, unrestricted NP license in Virginia along with an RN Compact license. Must have at least 2 years of experience managing complex care cases and proficiency with EMR systems.
