Key Responsibilities
Providers conduct comprehensive in-home health assessments for older adults and at-risk individuals to review medical history, medication adherence, and health risks. They collaborate with primary care physicians and other medical personnel to improve care quality and close care gaps.
Requirements
Candidates must hold a Master's degree and current Nurse Practitioner licensure with board certification in a relevant specialty. At least one year of NP experience and the ability to travel extensively for home visits are required.
