Key Responsibilities

The Social Worker provides specialized clinical counseling and support to patients with cognitive impairments and their families. They also act as a Patient Navigator to coordinate care, facilitate communication among providers, and connect patients to community resources.

Requirements

Candidates must hold a Master’s Degree in Social Work (MSW) and be licensed or eligible for licensure as an LCSW in Massachusetts. Experience working with geriatric or cognitively impaired populations in an interdisciplinary setting is preferred.
