Brighton/Suncrest Hospice is dedicated to providing exceptional care and service through a compassionate, supportive team that values communication, integrity, teamwork, and continuous growth.
Our Hospice Social Workers provide compassionate psychosocial support to patients, families, referring source and team members as they navigate the emotional, social, and practical challenges of serious illness and end-of-life care. You will serve as an advocate, resource, and trusted member of the interdisciplinary hospice team, helping patients and families receive the support they need throughout their hospice journey.
You will collaborate with an interdisciplinary team of nurses, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios.
This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.

Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
