Key Responsibilities
The Physical Therapist will assess, plan, and provide personalized rehabilitative services to patients in their homes to improve mobility and function. They are responsible for documenting patient progress, educating caregivers, and collaborating with the interdisciplinary care team.
Requirements
Candidates must be graduates of an accredited Physical Therapy program and hold a current, valid state license. A valid driver's license, automobile insurance, and CPR certification are required, with home health experience preferred.
