Key Responsibilities

The Physical Therapist will perform comprehensive patient evaluations, develop individualized treatment plans, and provide direct supervision and mentorship to Physical Therapist Assistants. They are responsible for ensuring high-quality, compliant care and maintaining accurate documentation throughout the patient's continuum of therapy.

Requirements

Candidates must hold a valid Physical Therapy license and have successfully completed an accredited physical therapy program. One year of home health experience is preferred, along with a valid driver's license, auto insurance, and CPR certification.
