Provide physician-prescribed physical therapy to patients in their homes to improve physical disabilities and functional levels. Participate in patient care planning and perform regular assessments and plan modifications every 30 days.
Requires a degree in physical therapy from an accredited program and a current state license. One year of experience in a home health setting and familiarity with Medicare OASIS documentation are preferred.
