Provide physician-prescribed physical therapy to patients in their homes to improve physical disabilities and functional independence. Participate in patient care planning and perform regular assessments and plan of care modifications.
Requires a degree in physical therapy from an accredited program and a current state license. One year of experience in a home health setting is preferred, along with knowledge of Medicare OASIS documentation.
