Key Responsibilities
The RN Case Manager will conduct comprehensive in-home nursing assessments and develop individualized plans of care for clients. They are also responsible for supervising Home Health Aides and ensuring high-quality clinical documentation and compliance with regulations.
Requirements
Candidates must hold an active New Jersey Registered Nurse license and have at least two years of recent experience in acute or home care. Proficiency in specialized clinical skills such as ventilator, tracheostomy, and wound care is required, along with current BLS certification.
