Job Description
Physical Therapist (PT) - Home Health
Serving patients across Carbon County | Per-Diem
Therapy Where It Matters MOST
In home health, you treat patients where real life happens: the stairs they climb, the bathroom they navigate, the kitchen they want to cook in again. You design therapy around those goals and watch patients regain strength, mobility, and confidence.
And here's our promise to you: when you come to work with us, we go to work with you. As a Home Health Physical Therapist, you'll never feel alone in this. You'll have a team behind you, a local owner who knows your name, and a system built to support you, so therapy can feel like therapy again. This is what real support looks like: an office that answers the phone, has your back, and celebrates the difference you make every single day.
