Key Responsibilities

Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.

Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.

Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.

Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition. Why Join Empath Health?

Requirements

Must hold an active physical therapy license in the state of Florida and a valid driver's license. Graduation from an approved physical therapy program is required, with home health experience preferred.
