About Enlight Hospice of VA

Enlight Hospice of VA is a Medicare-certified hospice in Richmond, Virginia. We give compassionate, team-based end-of-life care to patients and their families wherever they call home, including private homes, assisted living and nursing facilities. Our team focuses on comfort, dignity and quality of life, and treats the patient and family together as the unit of care. Office: 2809 Emerywood Pkwy Ste 260, Richmond, VA 23294.

  • POSITION
  • RN Case Manager - PRN (as-needed, per diem)
  • Reports to: Director of Clinical Services

Pay

$40 - $46 per hour, based on experience, plus mileage reimbursement at the IRS rate.

Schedule

This is a PRN (per diem) position. PRN means "as needed": you are not given a set number of hours each week. Instead, you pick up visits and shifts when our patient census or staffing calls for it, such as covering for a nurse who is out, helping with admissions, or filling on-call needs.

It's a good fit for nurses who want flexible, part-time hospice work, extra income alongside another job, or a way into hospice nursing. PRN staff may be asked to help with on-call and holiday coverage based on agency need. Patient visits are in the Richmond area, so you will travel to patient homes and facilities.

Responsibilities

  • Plan, coordinate and provide hospice care that follows each patient's plan of care
  • Do initial and ongoing comprehensive assessments, including medical history, physical exam, vital signs, mental status, pain and symptoms, and nutrition
  • Assess the home and family situation to find teaching needs, caregiver support and services needed
  • Develop and update the plan of care with the interdisciplinary group (IDG)
  • Work with the attending physician and Hospice Medical Director on changes in condition, orders and the plan of care
  • Give skilled nursing care, treatments and medications ordered by the physician, and report how the patient responds
  • Teach patients and families about the dying process, the plan of care, medications, diet and comfort measures, and guide them through healthcare decisions
  • Coordinate care with IDG members, contracted services and facility staff for continuity of care
  • Supervise Hospice Aides at least every 14 days, write the aide care plan, and give bedside care when an aide is not available
  • Guide and supervise LPNs as needed
  • Make death visits and help with discharge planning according to agency policy
  • Submit complete, accurate and timely clinical notes and visit schedules
  • Take part in IDG, case conferences, QAPI and staff development as scheduled
  • Keep up with current drug therapy and follow Medicare, Medicaid, state and accreditation requirements
  • Cover for other nurses and take on-call shifts, including holidays, as needed

Qualifications

  • Graduate of an accredited School of Nursing
  • Current Registered Nurse license in Virginia (or a valid multistate compact license)
  • Current BLS (CPR) certification
  • Valid driver's license and current auto insurance
  • Hospice, home health or case management experience preferred
  • Strong problem-solving, critical thinking, organization and time management skills
  • Clear verbal and written communication, and the ability to build trust with patients, families and the care team
  • Comfortable using a computer, tablet and electronic medical records
  • Able to complete all agency training requirements and work within safety and licensing standards

Physical Requirements

  • Regular travel to patient homes and facilities
  • Able to enter homes that may not be wheelchair accessible
  • Able to bend, stoop and reach, to safely help move patients up to 250 pounds, and to lift or carry up to 30 pounds
  • Hearing, eyesight and dexterity good enough to do physical assessments and patient care
  • High risk of exposure to bloodborne pathogens; standard precautions and PPE are required
