HOW YOU'LL MAKE AN IMPACT Your Role: Clinical pharmacist. Promotes safe, effective medication use at Tallahassee Memorial HealthCare, Inc. Provides carefully evaluated, evidence-based drug information (DI) recommendations to healthcare professionals; Supports specific medication use practices to enhance patient care quality, improve patient outcomes, and ensure the prudent resources use via formulary management, literature evaluation, and other medication use processes. Assists with implementing and following-up on medication use policies and formulary changes as part of a larger medication management program. Manages drug shortages promptly; Identifies alternative treatments, develops restrictive use protocols, and addresses formulary concerns. Precepts or provides advanced DI education and training to interprofessionals, pharmacy students, and residents. Utilizes excellent oral and written communication skills. Performs miscellaneous duties not expressly described herein. Reports To: Asst Dir Pharmacy Clin Program (JC: 020801) Supervises: None. WHAT YOU’LL NEED TO APPLY Required Education: Doctor of Pharmacy (Pharm. D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited pharmacy school. Required Experience: Two (2) years of hospital pharmacist experience. Human Resources (HR) may accept the successful completion of a postgraduate year two (PYG2) DI pharmacy residency in lieu of the required hospital pharmacist experience. Preferred Experience: Advanced training (e. g., residency, fellowship, and/or board certification). Effective communication skills. Leadership skills. Ability to work independently. Professional demeanor. Mature. Independent. Takes initiative. Required Certification/License/Registry: Licensed [or eligible for licensure (i. e., meets all statutory requirements for Florida licensure application)] as a pharmacist per Florida Statutes (F.
