- Provide evaluation
- Provide condition-specific support therapy, or when indicated, insight-based therapy
- Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Master’s degree in Mental Health Counseling or in Social Work
- Currently licensed as an LMHC or LCSW in the state in which you are applying
- Passion for geriatrics
- Experience in inpatient settings such as extended care facilities
- Experience with brief bedside assessment screens (cognitive, mood, etc.)
- Motivated by productivity-based positions
- Ability to navigate and document on an Electronic Medical Record (EMR) system

Key Responsibilities
Provide neurobehavioral health services, including evaluations and support therapy, to elderly residents in long-term care facilities. Coordinate comprehensive care plans with facility staff, primary care providers, and family members.
Requirements
Requires a Master’s degree in Mental Health Counseling or Social Work and current licensure as an LMHC or LCSW. Candidates must be able to work autonomously within an interdisciplinary team and navigate electronic medical records.
