Case Manager, RN, Full-Time - Main Campus

Key Responsibilities

The Case Manager coordinates and monitors patient treatment plans to ensure quality outcomes and efficient resource utilization. They also facilitate the continuum of care by collaborating with interdisciplinary staff and arranging appropriate aftercare for patients.

Requirements

Candidates must be a graduate of an accredited school of Nursing with a current Tennessee nursing license. A minimum of one year of broad clinical experience is required, and Case Management certification is expected within two years of hire.
