Key Responsibilities
- coordinate care with the patient’s primary care provider (PCP) and, when appropriate, other mental health provi.
- Provide patient education about common mental health and substance abuse disorders and the available treatme.
- Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, proble.
- Develop and complete relapse prevention self-management plan with patients who have achieved their treatment.
Requirements
- Two years of professional LCSW experience, preferably in an ambulatory care setting. Essential Functions:
- Support the mental and physical health care of assigned and referred patients.
- Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow
