Responsibilities

• Conduct comprehensive physical therapy evaluations and assessments for home health patients
• Develop and implement individualized treatment plans to restore function, mobility, and independence
• Perform therapeutic exercises, manual therapy, and rehabilitative techniques in the home setting
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care
• Educate patients and caregivers on home exercise programs, safety, and fall prevention strategies
• Maintain accurate and timely clinical documentation in compliance with home health regulations

Requirements

• Active Physical Therapist (PT) license in the state of Florida
• Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or DPT)
• Home health experience preferred but not required
• Valid driver's license and reliable transportation for home visits in the Fort Myers area
• Strong clinical assessment, critical thinking, and problem-solving skills
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently and manage a weekend caseload efficiently
