PREVENTIVE SOCIAL WORKER - SATS

Join CATHOLIC CHARITIES FAMILY AND COMMUNITY SERVICES as a PREVENTIVE SOCIAL WORKER - SATS in Rochester, New York.

Key Responsibilities

• Completes comprehensive assessments and develops, implements, and monitors individualized service plans appropriate to the client population served.

• Coordinates service delivery across systems, completing and facilitating required screenings and evaluations, including medical, educational, legal, vocational, law enforcement, and mental health services.

• Works collaboratively with Monroe County Department of Human Services, courts, and community providers, including attending court-related appointments and client service plan meetings, and may assist with transportation as needed.

• Supports clients during crises related to food insecurity, housing instability, abuse, domestic violence, and other emergent needs, while promoting connection to culturally responsive community resources.

Requirements

• Education: Bachelor’s or Master’s Degree in Social Work, Child Welfare or similar experience preferred.

• Credentials: Valid and clean NYS Driver’s License and reliable transportation.

• Experience: Two or more years of experience in the child welfare or community service fields preferred.

• Compliance: Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CFC’s corporate compliance & ethics program.

Benefits & Compensation

• Salary: $53K - $56K
