The Case Manager Registered Nurse will provide high-quality end-of-life care and develop individualized care plans for patients. They will also collaborate with interdisciplinary teams and maintain accurate clinical documentation in accordance with performance standards.
Candidates must be graduates of an accredited nursing school and hold a current nursing license in the state of practice. A minimum of one year of general nursing experience is required, with preference given to those with hospice or palliative care backgrounds.
