Key Responsibilities
Deliver comprehensive outpatient care to adult and senior patients within a team-based, whole-person care model. Manage complex medical conditions, coordinate specialist referrals, and contribute to clinical quality improvement programs.
Requirements
Requires an MD or DO degree, an active medical license, and Board Certification in Family, Internal, or Geriatric Medicine. Candidates must have at least 2 years of experience in value-based care or managing high-acuity geriatric patients.
