Responsibilities

• Conduct comprehensive in-home physical therapy evaluations and assessments for patients
• Develop and implement individualized treatment plans to restore function and improve mobility
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and gait training
• Monitor and document patient progress, adjusting care plans as needed
• Educate patients and caregivers on home exercise programs and injury prevention
• Collaborate with physicians, case managers, and interdisciplinary care teams
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements

• Valid Physical Therapist (PT) license in the state of North Carolina
• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Home health or outpatient physical therapy experience preferred
• Strong knowledge of home health guidelines and documentation requirements
• Excellent communication, time management, and organizational skills
• Reliable transportation and valid driver's license required
• Compassionate, self-motivated, and able to work independently in a home setting
