The Social Worker will perform in-person psychosocial assessments and collaborate with an interdisciplinary team to develop and monitor comprehensive care plans for elderly participants. They will also provide ongoing counseling, facilitate family meetings, and assist with resource referrals and end-of-life planning.
Candidates must hold a Master's degree in Social Work and possess BLS or CPR certification. Two years of social work experience in a hospital, nursing home, or community setting is desired, including one year working with the elderly population.
