Responsibilities

• What You’ll Do:
• Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals

Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults

Help patients restore functional mobility and safely perform daily activities with greater confidence and independence

Quickly integrate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes throughout your assignment

Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice
• Graduate of an accredited Physical Therapist Program
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults
• Willingness to travel to multiple TMC locations as needed
