Dove Healthcare is looking for a passionate, Registered Nurse Case Manager to join our dynamic team at our Skilled Nursing and Rehabilitation facility in Lodi.

If you are a registered nurse (RN) who desires to take the next step in the career ladder and use your experience to mentor, train, and educate a team of caregivers, while coordinating and overseeing the care of your patients, the role of nurse case manager may be your next calling. At Dove Healthcare, you will work with employees, patients, family members, and other professionals to coordinate all aspects of providing person-centered care.

What You'll Do

supervise the day to day activities of assigned residents and employees coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence review and audit compliance with the patient's plan of care and changing needs provide a safe, comfortable, and home-like environment participate in an on-call rotation
