Job Details: Job Location: Hospice Savannah Inc. - SAVANNAH, GA, Salary Range: Undisclosed, Job Summary The Physical Therapist provides comprehensive physical therapy services to PACE participants, including assessment, care planning, intervention, education, and ongoing monitoring to support functional mobility and safety. This role collaborates closely with the Interdisciplinary Team (IDT) to ensure participant needs are identified, addressed, and incorporated into individualized care plans. The Physical Therapist also contributes to care coordination, documentation, quality initiatives, and participant and caregiver training within the PACE model of care.

Qualifications

Minimum Qualifications Master’s degree in Physical Therapy (Required) Valid driver’s license and reliable transportation One year of experience working with a frail or elderly population, or the ability to receive appropriate training upon hire (Required) Must not have been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under Title XX (Required) Must not have been excluded from participating in Medicare or Medicaid programs (Required) Must not have been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse (Required) Competencies, Preferred Knowledge, Skills, and Abilities Preferred Doctorate degree in Physical Therapy (Preferred) Preferred Minimum 1-year work experience as a Physical Therapist (Preferred) Preferred experience working in Electronic Health Records (EHRs) (Preferred) Excellent verbal and written communication skills. Strong attention to detail. Knowledge of and ability to use computer or typewriter keyboard Ability to use other software as required to perform the essential functions of the job. Proficient with Microsoft Office Suite including Word, Excel, and Outlook. Physical Demands and Work Environment This role operates in a combination of clinic, PACE center, community, home, and facility environments. Duties require frequent standing, walking, bending, reaching, transferring participants, and performing therapeutic interventions. The employee must be able to lift, push, and pull up to 75 pounds in connection with mobility equipment and participant assistance. Entering participant homes or facilities may involve navigating stairs, uneven surfaces, tight spaces, and varying environmental conditions. Work may also include exposure to outdoor weather during participant visits.

The position requires the ability to read, review, and enter information into the electronic health record (EHR) and other computer systems, including prolonged periods of viewing a computer monitor and performing close visual work.

Keyboarding, data entry, and documentation tasks require fine motor skills and sustained concentration. The employee must be able to communicate effectively in person, by phone, and through electronic platforms. Position Type and Expected Hours of Work This is a full-time, non-exempt position. Standard hours are Monday–Friday, 8:30 a. m. to 5:00 p. m., with occasional schedule adjustments based on participant needs, home visit requirements, or operational considerations. Travel Travel is required for this role. The Physical Therapist regularly travels within the PACE Georgia service area for home visits, facility visits, participant assessments, care coordination, and meetings. Occasional travel for training or organizational functions may also be required.
