The Physical Therapist performs clinical assessments, including OASIS, and develops individualized plans of care for geriatric patients in their homes. They also provide direct treatment for various disorders, supervise therapy assistants, and maintain accurate clinical documentation.
Candidates must hold a current Physical Therapist license and possess strong clinical assessment skills. Prior experience in home health care is preferred, along with familiarity with Homecare Homebase EMR software.
