Physical Therapist (PT) – Mobile/Home-Based Care

Key Responsibilities

The physical therapist will travel to patients' homes to perform comprehensive evaluations, develop individualized plans of care, and provide skilled therapeutic interventions. They are also responsible for monitoring patient progress, educating caregivers, and maintaining accurate clinical documentation.

Requirements

Candidates must hold a current and valid Texas Physical Therapist license and have graduated from an accredited Physical Therapy program. Strong clinical reasoning, effective communication skills, and the ability to work independently in a mobile environment are required.
