The Care Transitions Manager directs and coordinates resident care services to ensure successful transitions from skilled nursing to home environments. This role also provides leadership oversight, manages rehabilitation vendor relationships, and supports the Director of Nursing.
Candidates must hold an active RN license in good standing with at least two years of nursing experience and one to three years of supervisory experience. Proficiency in state and federal skilled nursing regulations and strong communication skills are required.
