Job Description
About AllCare. ai: AllCare. ai delivers coordinated care to seniors in assisted living and skilled nursing communities across Southern California. Our services include Primary Care, Psychiatry, Podiatry, Cardiology, Diagnostics, Wound Care, and Care Management, supported by a 24/7 medical concierge.
Our platform handles documentation and compliance so providers can focus on high-quality care. Clinical services are delivered through our affiliated medical group, Dazzle Health, P.C.
Role Overview
We are building an in-house Specialty Pod so our senior patients can see a specialist quickly, without long waits or insurance roadblocks. We are seeking a part-time Nephrologist (MD/DO) to lead its kidney care line.
Most visits are by telehealth, with a Care Partner at the patient's bedside, plus optional mobile visits in the community. You will set the care plan; a nurse practitioner, a registered dietitian and our care team carry out the monthly follow-through.
This is a 1099 independent contractor role with Dazzle Health, P.C. You will be credentialed and bill under Dazzle Health's group NPI, so we handle payer enrollment, billing and collections.

Supervise and guide a nurse practitioner who handles follow-ups, kidney disease education and monthly care management
- Review blood pressure remote monitoring data and lab trends with the care team
- Help shape the kidney care protocol: referral criteria, lab panels, visit cadence and blood pressure targets
- Coordinate with our cardiology and primary care teams on patients with heart and kidney disease, hypertension and diabetes
- Keep documentation accurate, timely and compliant
- Active California medical license (MD or DO) in good standing
- Board Certified or Board Eligible in Nephrology
Active DEA Registration
- Active Medicare enrollment, or willingness to enroll and reassign benefits to Dazzle Health, P.C.
- Comfortable with telehealth visits
- Willing to supervise a nurse practitioner
- Experience caring for older adults with CKD preferred
- Medi-Cal enrollment preferred
Compensation
- $100 per new patient visit
- $75 per follow-up visit
- $25 per completed eConsult
- Telehealth or in person; paid monthly; reported on a 1099-NEC
Schedule Part-time and flexible. You can start with a few telehealth sessions a month and add more as the patient panel grows. Scheduling is built around your existing practice.
Benefits
- Flexible, mostly remote schedule that fits alongside your current practice
- Additional revenue without the overhead: we handle credentialing, scheduling, billing and collections
- Care team support: NP, dietitian, Care Partners and charting assistance
- Patients arrive with labs drawn and records gathered before the visit
- Opportunity to help build a specialty kidney care program from the ground up
- Expand specialist access for seniors who wait months for a nephrology appointment
