Key Responsibilities
The Licensed Master Social Worker assesses the psychosocial needs of hospice patients and their families to develop and implement effective care plans. They also provide supportive counseling, facilitate community referrals, and participate in interdisciplinary team meetings to ensure comprehensive patient support.
Requirements
Candidates must hold a Master's degree in Social Work and possess a professional licensure as a Master Social Worker in the state of employment. Additionally, clinical experience or formal training related to death, dying, and bereavement is required.
