The Behavioral Health Case Manager serves as a point of contact and advocate for members, providing face-to-face and virtual services to support their recovery journey. They are responsible for developing, implementing, and monitoring person-centered individual service plans while maintaining accurate clinical documentation.

Requirements

Candidates must possess at least a high school diploma or GED combined with relevant experience or certifications, such as Peer Support Specialist or Medical Assistant. Additionally, applicants must be at least 21 years old, hold a valid driver's license with a clean driving record, and pass a drug screen and background check.
