Behavioral Health Patient Navigator

Key Responsibilities

This position assists referred patients from Psychiatry, Emergency Department, and other units in achieving follow-up treatment discharge goals by educating them on accessing community mental health resources and services. Responsibilities include meeting with patients, assisting in connecting them with referred resources, conducting timely follow-up calls post-discharge, documenting plans, and coordinating care needs with providers.

Requirements

Minimum qualifications require excellent interpersonal and communication skills, along with the ability to adapt quickly when needed, and knowledge of available behavioral health community resources. Preferred qualifications include three years of experience working with behavioral health patients in a care setting, post-high school education in a related field, and experience with behavioral health discharge planning.
