Key Responsibilities
- Conduct physical assessment/histories to determine health problems, homebound status and follow plan of care/treatment plan in collaboration with the physician.
- Observe patient's condition to monitor response to treatment and communicate/document changes to physician.
- Counsel and teach home program to patient and significant other /caregiver to accomplish goals.
- Formulate appropriate discharge plans.

Requirements
Candidates must possess a current Physical Therapist license in the State of Florida. A minimum of one year of experience as a PT is required.
