The Triage Registered Nurse performs comprehensive patient assessments via phone to determine hospice needs and develops individualized care plans. They act as a Case Manager to coordinate patient care, administer prescribed treatments, and provide health education to patients and their families.
Candidates must hold a valid California RN license and possess at least two years of hospice experience. Proficiency with Home Care Home Base software and current CPR certification are required.
