Key Responsibilities
Facilitate the care of patients and their families throughout their hospital admission or outpatient visit. Coordinate discharge planning and provide psychosocial assessments to address evolving patient needs.
Requirements
Requires a Master's degree in Social Work and a current State of Wisconsin social worker license. A minimum of two years of experience in an acute care setting is preferred.
