Key Responsibilities

Provide comprehensive psychosocial services to terminally ill patients and their families, including assessments and care planning. Act as a case manager to coordinate support, counseling, and community resources while participating in interdisciplinary team meetings.

Requirements

Requires a minimum of two years of social work experience in a healthcare or social services setting. A Bachelor's degree in Social Work is required, with a Master's degree preferred, along with a current Social Worker license and a valid driver's license.
