Key Responsibilities
The Social Worker will assess the psychosocial needs of hospice patients and their families while providing counseling, crisis intervention, and emotional support. They will also coordinate community resources, assist with advance directives, and maintain accurate clinical documentation.
Requirements
Candidates must hold a master's or bachelor's degree in social work and possess a current professional license. A minimum of one year of experience in health care or hospice is required, along with a valid driver's license and insured vehicle.
